Understanding Estrogen Cream

Know the treatment.
Ask better questions.

Topical estrogen may be considered for specific menopause-related symptoms. The application site, formulation, and dose affect its use and risks.

How it works

Estrogen acts on hormone receptors. Local and systemic products differ; the correct formulation depends on symptoms and individual circumstances.

Safety & suitability

Certain cancer histories, unexplained bleeding, blood clot history, and other conditions can affect suitability. Systemic estrogen may require a progestogen when a uterus is present.

Bring to your care conversation

  • Is local or systemic treatment appropriate, and would I need endometrial protection?
  • What is the exact formulation, and which approved alternatives should I consider?
  • What side effects, interactions, and follow-up should we discuss before a decision?

Research, in perspective

What the evidence says.

Indication-specific evidence

The route matters as much as the hormone.

Approved systemic skin gels and local vaginal estrogen products serve different menopause-related needs. Evidence for one route cannot establish the effects of a cream with an unspecified concentration or application site.

FDA's summary of the NASEM review found limited high-quality evidence supporting compounded bioidentical hormone therapy. A compounded cream should not be presented as safer or more effective than approved alternatives.
