Understanding Low-Dose Naltrexone

Know the treatment.
Ask better questions.

Low-Dose Naltrexone, often called LDN, describes off-label use of naltrexone at lower doses than approved addiction-treatment regimens. The pictured label does not specify dose or delivery form. This listing does not establish benefits for any individual condition.

How it works

Naltrexone blocks opioid receptors. Proposed additional effects of low-dose use remain under investigation, and a proposed mechanism does not prove benefit for chronic pain or inflammatory conditions.

Safety & suitability

Naltrexone can block opioid pain medicines and precipitate withdrawal in someone using opioids. A clinician must review current medicines, opioid exposure, liver health, and other risks. Do not start, stop, or change medicines based on this page.

Bring to your care conversation

  • What evidence supports use for my diagnosis, and how does it interact with opioid medicines?
  • What is the exact formulation, and which approved alternatives should I consider?
  • What side effects, interactions, and follow-up should we discuss before a decision?

Research, in perspective

What the evidence says.

Limited human evidence

Condition-specific research. Mixed findings.

A 2024 randomized trial in 99 women with fibromyalgia tested oral low-dose naltrexone against placebo for 12 weeks. It did not demonstrate superior pain reduction.

That finding does not settle every dose or condition, but it does not support a broad pain-relief promise. Standard-dose approval for addiction-related indications does not establish approval of low-dose compounded formulations.
